Provide Documentation After You Pay

Upon leaving the payment portal, you will be redirected to a form to complete that provides us with the information we need to give you access as a prescriber in our application. It requires that you upload a valid government ID and a recent utility bill. There is also a terms of service agreement that you need to print out, sign and return after purchasing a subscription.If you don't have a scanner or way to upload these documents, you can fax them to us via the number provided on the form. Once we get all this information from you, we will proceed with setting up your access to the application. Most accounts be be live in 24-48 hours.
